Coordinate all phases of nutrition care including nutrition assessment, care planning, education, counseling and monitoring of patients. Perform duties in support of and in compliance with performance improvement plans, licensing, accrediting, and regulatory agencies.
In-depth knowledge about current nutritional management of diabetes and other common chronic medical conditions. Good communication and organizational skills. Ability to develop a collaborative, therapeutic alliance with individuals. Computer skills (MS Word, Excel, Internet and Email). Minimum of 1 to 3 years’ experience providing diabetes education. Spanish language fluency a plus.
Registered Dietitian with the Commission on Dietetic Registration (CDR), active license with the NC Board of Dietetics/Nutrition; Certified Diabetes Educator preferred, although candidates experienced in working with people living with diabetes will be seriously considered.
