Typical work week is Monday-Friday from 8:30 a.m. to 5:30 p.m.; occasional night and weekend work may be needed to meet the needs of the department.
This position will require day and overnight travel in the eastern NC area with responsibility for telecommuting from areas accessible to I-95 (eastern NC). The employee will also be expected to travel to the UNC Asheville campus periodically.

This position is funded by a three-year federal grant from the Administration for Community Living. The purpose of this project is to increase the sustainability of Chronic Disease Self-Management Education (CDSME) programs for older adults and adults with chronic conditions and to improve pathways for Medicare reimbursement of these programs including Diabetes Self-Management Education and Support (DSMES) programs and Medical Nutrition Therapy. The Diabetes Project Manager will be responsible for managing grant deliverables, facilitating partner accreditation through American Association of Diabetes Educators and expanding reimbursement pathways for these programs, convening meetings, managing subcontracts, preparing reports, and coordinating communication among partners and the funder. The employee in this position will be expected to work collaboratively as a member of the NC Center for Health and Wellness (NCCHW) team.
This position will require day and overnight travel to support Area Agencies on Aging across NC. Due to the work related to this position, the North Carolina Center for Health and Wellness (NCCHW) seeks applicants willing to live in the eastern NC area with responsibility for telecommuting from areas accessible to I-95 (eastern NC).
